Pharmacist(Clinical Spec)
Evaluates and monitors medications for appropriateness of therapy and patient's response to therapy based on relevant therapeutic endpoints by means of patient interviews, physical and functional assessment, and diagnostic tests. Requests consultations or referrals to other health care professionals to render appropriate comprehensive care to the patient. Prescribes and modifies medication regimens in a safe, effective, and economical manner. Provides these collaborative drug therapy management services in accordance with the individual's approved scope of practice and prescriptive authority. Coordinates and manages consultative therapeutic drug monitoring service s as requested by providers on selected drug therapies. Evaluates drug therapy through direct patient care involvement, with clinical assessment and objective findings relating to patient's responses to drug therapy and communicating and documenting those findings, interventions and recommendations to appropriate providers and in appropriate records (e.g. patient's medical record). Documentation is done in a timely manner and consistent with the departments established standards of practice. Prescribes, continue s, modifies, or discontinues drug therapy for both inpatients and outpatients only in accordance with the individual's approved scope of practice and prescriptive authority. Must have 1 year of experience equivalent to the next lower grade level. 1. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following:
designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
